Please Note:
- All payments must be received no more than 7 days after end of registration period.

- Indicate on the check the name(s) of the registered person(s)
- We will only accept payment for registration by personal or cashiers (bank) check, or money order. No credit cards or cash will be accepted either by mail or for the on-site registration.
- Registration will not be confirmed until full payment is received.
- Cancellation/Refund Policy: Full refunds will be given if requested by January 17, 2005. There will be no refund if canceling within 7 days of the conference.
